
Diabetic carbuncle is a severe deep skin and soft tissue infection commonly seen in individuals with poorly controlled Diabetes Mellitus. A carbuncle is formed by the coalescence of multiple infected hair follicles, resulting in a large painful inflammatory swelling with pus formation involving deeper layers of the skin and subcutaneous tissue. Due to impaired immunity, poor circulation, elevated blood glucose levels, and delayed wound healing associated with diabetes, patients are more susceptible to recurrent skin infections and extensive tissue involvement.
The condition is most commonly caused by bacterial infection, particularly Staphylococcus aureus. Diabetic carbuncles commonly occur over the back of the neck, shoulders, back, thighs, and buttocks. Symptoms often include severe pain, tenderness, redness, warmth, swelling, pus discharge, fever, fatigue, foul-smelling drainage, and surrounding cellulitis.
In Ayurveda, diabetic carbuncle can be correlated with complications arising from Madhumeha associated with vitiation of Pitta and Kapha dosha, impaired tissue metabolism, accumulation of toxins (Ama), blood vitiation, and weakened tissue resistance.
